Reporting CPIM Hours

The TOS has established new reporting requirements for CPIM hours.  Attendees will now fill out the CPIM Certification of Hours Form provided to you during one of our events.  You can then scan the completed form and email it directly to CPIM at CPIM@tos.ohio.gov or leave it with us at the end of the last session and we will forward it to CPIM on your behalf.  The form must be returned to CPIM within 7 calendar days of completing the courses to receive credit. Please reach out to oapteducation@gmail.com if you have any questions on how to submit CPIM hours.
